CIsco CAD 7.2.1 SR2 ICM Silent Monitor error and no audio when monitoring

At the supervisor Applet click on a Agent talking to a Outside party CAN hear just fine. Then try monitoring a agent to agent call get silent monitoring error box come up and no audio. There has got to be something up with doing silent monitoring agent to agent. Can get it to happen every time. Like I said it works when they recieve a inbound trunk call or are calling out I can silent monitor just fine. All our agents use Ext. mobility, and Desktop Monitoring through the daisy chained phones to their PC's

Anybody run into the agent to agent no silent monitor issues
