Some things to check:

Is the phone getting an IP address from DHCP?

Is it getting TFTP option?

Is it configured in CUCM/CME - you did not specify which system you have

Is CUCM definde with IP or hostname (could be DNS issue)

What do you see  in the Status messages on the phone?
