I have installed CRS 5.0(2)_Build064. I noticed that CRS Engine->Subsystem Manager->Enterprise Server Data Subsystem is OUT_OF_SERVICE. Also CRS Engine->Manager Manager->Application Manager is PARTIAL_SERVICE (then I can't listen to prompts when dial the application trigger).
I restart CRS Engine, reboot server, but didn't work.
Did you ever get your Partial_Service issue resolved with the Application Manager? I'm running the same version of CRS as you and I have both the Application Manager and Unified CM Telephony Subsystem in Partial_Service status.
