IPCC Express agents log out randomly

Running IPCC Express Enhanced 4.0 and CCM 4.0. Agents phones are logged out randomly. In realtime reporting, the agents show that they are logged in and ready. Phone says they are logged out. No errors in the Application Events log. Any ideas??? Thanks in advance.

Same issue here but with version 5.

only pattern I have noticed is once an agent gets logged off it will continue to happen to that agent for the rest of the day.
