IVR/IPQM upgrade from 2.2.5 to 3.1.3

We have IP IVR/QM 2.2.5 and we are upgrading to 3.1.3 and moving at the same time to a new server.

What is the best procedure to do it?I'm not clear with Cisco Doc's.

Have anyone done this before?
