ASA5510 access denied /pass recovery /t...

ASA5510 access denied -I run a password recovery, then I can login, works for a day or so then I am locked out again. latest code 7.2.222. I had this problem a few months back updated code seem to fix it but now it's doing it again.

Is your asa rebooting during before it no longer works? Are you saving the running config to startup after you make the change?
