The process is quite similar whether the packet is going to a destination within the same enterprise network or to a destination accross the Internet.
A PC generates the packet and sends the packet to the PC's default gateway. The gateway receives the packet, examines the destination address, does a lookup in its routing table and using either a specific route found in the routing table or using its default route decides which interface leads toward the destination, and then forwards the packet out the outbound interface to the next hop router. The process is repeated on however many routers are in the path: receive the packet, examine the destination address, do a lookup in the routing table, using either a specific route or a default route make the forwarding decision, send the packet out the outbound interface to the next hop router. Eventually the packet makes it to a router which examines the destination address and that address is on a connected subnet. This router then forwards the packet to the destination host.
