You should only need to pull the LWAPP/CAPWAP recovery image from cisco.com. Then you can either use the upgrade tool or the archive download command to pull and load the software onto the AP.
I'm order to help us understand the issue, you need to console into that AP and post the output so we can see if your performing the upgrade properly.
There are any docs that explain the sets and some videos. Make sure your using A compatible TFTP server. I use 3CDeamon and have not have any issues. Also make sure that your firewall is disabled on you machine as tftp is usually being blocked.
