Cisco FWSM nat not working for some of the ip address

In FWSM module in Cisco 6509 router with IOS version 4.0(8), some of the ip addresses are not natting properly intermittently.

Some of the IP addresses are working properly.

As the issue is happening randomly, we have to run "clear xlate" every time the issue happens.

As I am not able to find a published document on the same.

Can any body throw some light on this.
