IPS 4240 won't boot

After attempting to install a newer system image, I receive the following output:

Launching Bootloader...

Default configuration file contains 1 entry.

Searching / for images to boot.

No images in /

Error 15:File not found

Unable to boot an image

I can tftpdnld the image successfully, but it does not come up.  Does a path to an image file need to be specified?  Any help would be really appreciated.

Hi.

this could possibly be a bad disk issue. try to re-image the sensor and if that fails, a hardware replacement is needed.
