IPS4260 - PXE-E61: Media test failure, check cable

Can't boot the appliance (IPS-4260-4GE) at the first

startup sequence !

That is a generic error that indicates that the device is trying to boot from the network and failing. you might try to 'break' in during bootup and use tftp to get the latest image.
