7941g cannot upgrade - "frozen"

The device can get an IP, is "pingable", but only shows "Upgrading" -- doesn't even start getting a load.

Performed factory reset, tried separate voice VLANs, even switchports that have no issue with TFTP connectivity.

Is it a brick? Thanks for your help on this issue!

No.

connect it directly to PC with TFTPD32. Configure TDTPD32 with dhcp option 150 and load there the FW files. You will be able to observe the log and correct anything as necessary.
