Attendant Console problems

We installed Attendant Console Client. After you enter your username and password it displays "Initialization of call control failed" and the program opens. It will not let you make any calls and shows a question mark next to the line # for the Attendant Console. How do you get it to work?

Have you associated the user's phone with the ac user in CCM?
