Call Park with Attendant Console in CUCM 5.1

Two problems exist.

1. Receptionist can place a call in park and the call can be retreived by phone. However, she cannot retreive the call herself using AC. She gets"failed to make new call" and hears "your call cannot be completed as dialed"

2. This one is intermittant. Some parked calls do not get removed from the AC when they have been picked up. They will remain there until she logs out of AC.

Any help would be appreciated.
