if the issue is intermittent then try to isolate first if it is related to certain phone models, certain site, certain device pool etc. Try upgrading the firmware of the affected phones or downgrade it if it was working fine before and see if it helps. Detailed callmanager service traces for a call with failed pick up would be needed to proceed further if firmware change or server reboot does not help.

I used Cisco 7861/7821 .

all phone configured Identically .

Then proceed as per the other points that i mentioned.

when you say"cant pick up: do you mean the phone rings, but when the answer button is pressed to handset lifted, there is dead air?

or the phone just doesnt ring?

when I try to pickup from another extension I got error: "No call Available for Pickup "

check the partition, the Pickup group is in.

To confirm, you have the Hunt Pilot configured with the Call Pickup Group and the extensions you are testing with are in the same Pickup Group?

yes

There are couple of listed bugs about the same problem but for earlier versions of CUCM. One of them is CSCtl86234. Not sure if you are hitting one of them but worth opening a TAC case or navigating through cisco bug toolkit.
