CAn callmanager express support 5ess call by call? If so, what would have to be coded?

Sure it can, 5ESS is an AT&T ISDN switch-type. I am assuming you have a PRI, in order to configure a PRI with CME, do the following:
1. Define the ISDN switch type in the global configuration.
IOS-GWY(config)#isdn switch-type primary-5ess
2. Configure the T1 controller for ISDN PRI signaling.
IOS-GWY(config)#controller t1 1/0
IOS-GWY(config-controller)#framing esf
IOS-GWY(config-controller)#linecode b8zs
IOS-GWY(config-controller)#pri-group timeslots 1-24
3. After configuring the pri-group command, the D channel (interface serial 1/0:23) and the voice port (voice-port 1/0:23) are created automatically by the router.
4. Configure dial-peers

I think that "call by call" is just a market name for ISDN PRI, nothing else.
