Can not reset My cisco 7931 IP PHONE

I have my CME that is in 3945 Router, when i register Ip phone some of 7931 registered and working but some after reseting them they stop in upgrading state.

Did you mentioned phone type

Did you mentioned phone type correctly under ephone ?

Please send accross CME config , ephone config and debug tftp events.
