CER 8.01 - Phones in "Unlocated Phones" pool after switch replacement

I'm running CER 8.01 with CUCM 7.15.

A switch died at a remote location, and it's been replaced with a WS-C3560X-48P switch.  It has the same IP address as the previous switch, and the same SNMP settings as well.  All phones attached to this switch (about 10) are dumped into the "Unlocated Phones" pool in CER.  The phones were dumped into the correct ERL on the previous switch.  The phones are directly attached to the switch, they show up in CDP, they're all 7940 phones, I can ping the switch from the CER server, other switches in the same subnet are reporting the phoes correctly, this has been going on for weeks.  I have manually ran the "switch-port & phone update" several times.  I turned on "debug SNMP packets" and when I manually run the "switch-port & phone update" the following is what the switch log shows:

The other switches at the location (and same subnet) respond with loads of information.  The switch is running IOS Version 12.2(53r)SE1.

Any ideas?  I went over the admin guide, and was able to rule out the basic reasons this would be happening.
