Sometimes, when CFwdALL on phone is deactivated, that forward gets stucked on phone and stays active. But there in no notification on phone that phone is forwarded. Solution is to manualy set/remove forward for that line (via ccmadmin webpages).
I've found similar feature on CCM 4.1(3)sr2. TAC recommended upgrading to latest 4.1(3) service release as there are three known bugs that can cause this issue. I haven't had time to implement new service releases so I'm not sure yet, if it solves the problem.
