I have a CME4.0(0) setup with 7940 and 7960 phones, when a call comes through the T1 PRI with DID the called party answer, then is asked to transfer the call to X person. If X person doesn't not answer the call after some rings, the transfer is canceled and the original called person goes back to the caller. At this point, the phone that was trying to be reached with the xfer is still ringing and the called person is still hearing the ring on the background of the conversation.
no call-forward pattern is configured.

Can you try 12.4(11)XJ4 that is very stable, see if still happens.
