CME 4.1 outbound dialing not working

I have a strange problem at a site where inbound dialing into the CME 4.1 works fine. 3 POTS lines into an FXO card. Outbound dialing either gives fast busy or "number has been disconnected". If i do a debug on dial peer it looks like everything is working. Tested from the telco demarc and works fine without CME in the mix. Anyone run into something like this before? CME 4.1, IOS 12.4.15T. Thanks in advance

Make sure your dial-peers are correct and you're sending the necessary digits, if they're correct do a debug voip ccapi inout to find who and why the call is dropped
