For example, in the case of the secretary who knows the boss is busy, is there a way for her to send/transfer the caller directly into the boss's VM box, without the ringing of the phone first?

If I'm not worng, this feature now appeared in CME 4.3
However, remember that some callers are able to tell from the ring pattern that they have been transferred directly and can complain about the low priority given to their call.

Just to add a note to the great info from Paolo and Matt (you guys Rock!). Here is another method (if you don't have CME 4.3), the steps required are nicely detailed in this good Tech Note;

Here is another simpler was of doing this;
ephone-dn 1
number 8.... (assuming that the ext length is 4 and number 8 will be used to directly xfer to VM)
call-forward all
translation-profile outgoing XferVM
voice translation-rule 8
rule 1 /^8\(.*\)/ /\1/
voice translation-profile XferVM
translate redirect-called 8
