I just installed CME 4.0 and we are experiencing some issues. They have a Integrated T1 with 6 channels of voice. The voice channels are set up with fxo-loop-start. The problem occurs when an outside caller hangs up the phone. The Telco says that the cut off on disconnect is on the line and working. However, then the CME receives the disconnect, the end point will receive dialtone, as if the line is now seized by the phone.

Good observations. I think the sentence "to be on denial" started from some telco.
There are exceptions anyway, a little anecdote that I want to tell here. Some years ago, I noticed I had voice cuts at home, so called telco (just not to make names, it was pacbell) and complained. They said all was fine and after I insisted, some day later a kind young lady calls me back saying that they ran checks on the line and I should check my phones. In deep disbelief I proceeded to do so and found a cheap phoneset that I was very fond of, was the culprit.
