dx650 ringback issues and occasional failed calls

I believe these are actually SIP issues but i'm having some outbound calling and ringback issues on my DX650. When I place an outbound call i hear nothing but silence. it says "calling" on the screen and in q931 logs I can see a channel reservation and outbound call attempt. 80% of the time the call will fail. What happens is the phone app on the DX650 returns to the dialer without any prompting or notification, on the gateway I see the call end, and the remote caller never received a ring or was able to accept the call. What are some things I can look at or check to resolve this?
