Hi,
I have implemented the following:

inter-branch calls work 100%. Problem is that when remote site phones out (PSTN breakout at HQ) the audio is extremely soft. Any ideas?

Have you tried adjusting gains and attenuation under voice port ?
Also very important, check you optimal impedance setting:
