In a lorge environment, IT teams are there in different locations and different levels, In this case, If we want to know who created the particular extn in CUCM or removed the Extn in CUCM, What is the way apart from the RTMT log file?

Without a third-party tool, there isn't a way.

There are audit logs (http://www.netcraftsmen.net/resources/blogs/audit-logs-on-cucm.html) on certain CUCM versions that can be used in addition to the Cisco Tomcat logs (localhost_access) to see the username and IP address of who made the change specified in the audit log.
