Issues transferring calls in CUCM 9.1.1

We installed a new phone system a few months ago and we have a few uses that have issues transferring a call. The call comes in, they hit the transfer button, wait for the line to ring, and hit transfer again. This is where the issue comes in, as soon as they hit transfer the second time, the system drop the call to the third number and places the original call on hold for them to resume.

We have CUCM 9.1.1 with 7945G and 7965G IP Phones. This is an intermittent issue, it does not happen all the time. Has anyone else had this issue? Is there a resolution?
