MeetMe Conference Problem

I have a single CCM 4.0.2a cluster and I am experiencing a strange problem on this one specific MeetMe extension. It seems like the extension is stucked in active in CCM from the last time it was used. I am getting dead air when calling the extension or get a busy signal when try to initial a MeetMe call with this extension. And all other MeetMe extensions are working fine.

Here is what I have done so far but all didn't seem to help.

- I've restarted DBL Monitor and RIS Data Collector services on all servers

- I've deleted the MeetMe extension and recreated it.

And users have been using this MeetMe extension for years so we know this extension was working before.

Has anyone run into this issue before ?

Any help or advise is highly appreciated !!

Remember he very first party press Meetme Softkey then you enter MeetMe Number.

Following parties only dial MeetMe number.

If using 4.0.2a I would apply latest SR or ES due to:

If an upgrade is not an option, normally we need to match DDTS against SDI(CCM)|SDL detailed traces before an upgrade recommendation so please go ahead and upload them, I can take a look.
