How is your vocera server integrated to the Call Manager system? do you have FXO ports on the cisco VOIP gateway that is trunked to the vocera server or IP trunk to vocera? also check if you have multicast routing enabled in your Network if the vocera server resides in different network to CCM.
Do you have H323 gateway or MGCP?. try adding "ccm-manager music-on-hold" on the gateway and test.
