I do not know if this possible, I need to assigned one softkey to Personal Address Book, and assign default ID and PIN for this PAB for all users.

You can create a non-Enterprise Subscribed copy of the Phone Service and subscribe this to a Service URL PLK on the phone. You're out of luck when it comes to pre-populating the username and PIN though. Each end user will need to enter their credentials the first time.
