I am trying to find the logs that show when a 7960 phone was added or modified in CCM4.1(2). Does anyone know where this log file might be located? Thanks in advance.

If you are using MLA you can find them under Program Files|Cisco|Trace|MLA
Otherwise I only would check IIS logs.
Or start thinking in CUOM or other Cisco Management Tool for Access.
