"I didn't recognize that as a valid entry" when dialing directory number from outside

Hey all - I'm having an issue provisioning a phone for an employee and I cannot, for the life of me, figure out what the issue is. I have setup a 7941 for the user with an extension of 612. I am able to dial his number internally, I am able to dial out externally from his phone and dial from his phone to any number internally. The problem is when I call our office the attendant tells me that this is not a valid entry. 

Has this happened to anyone before? I cannot figure out what the issue is....

And who is the attendant????? and what is the call flow????

I would assume CUC from what you're saying, but you need to provide that information.

Is there a mailbox with extension 612? By default Unity will not route to numbers if they do not have mailboxes.
