You'll use a SIP trunk on CM. You may need a CUBE between CM and the service provider, depending on what the service provider wants to see. IF they need early offer on outbound calls or DN registration, you'll need a CUBE.
Here is a guide for skype to a uc500. It walks through the CCA config, but you should be able to reverse engineer a CUBE config from that, if you know what you're doing.
Your service provider may have sample configs for Cisco gear, too. Otherwise, start configuring and watch SIP messages to see what happens, and tweak/act accordingly.

No one is going to be able to just throw you a working configuration for something like this. You're going to need to do some research and learn about how thw products work and interface with SIP peers.
First, check out these documents.
CUBE Configuration Guide:
http://www.cisco.com/en/US/docs/ios/voice/cube/configuration/guide/12_4t/vb_12_4t_book.pdf
CUCM SRND: SIP Trunks
http://www.cisco.com/en/US/partner/docs/voice_ip_comm/cucm/srnd/8x/trunks.html#wp1044916
If you aren't familiar with the SIP protocol, you'll want to brush up on that, too:
http://www.cisco.com/univercd/cc/td/doc/product/voice/c_ipphon/sip7960/sadmin31/bsipcflw.htm
From there, give configuration a shot, and if you have a specific issue, feel free to ask.
