Two issues. Prob more related to routing.
Transfer: When pstn callers call into HQ and the HQ transfers them out to the branches they cannot hear the pstn people
Fowarding: This is totally random when it happens. If I have my phone set to foward to my cell and a PSTN caller calls in my cell phone rings and I pick it up but the PSTN caller still hears ringing.

For transfer are you binding the MGCP/H323 call signaling and media path to a loopback address on the ingress router?
For the forward:
Does the call connect? What is the Latency like?

Are you using G.729br8 with MGCP?
If so, you're probably hitting this bug: CSCsy10653
The workarounds listed are very easy to implement.
