Have a 2821 running CUCME 7.1
VIC3-4FXS/DID is installed to provide analog connection for a fax machine however none of the FXS ports are providing dial tone. I've had a user plug an analog phone directly into each port of the FXS card and they don't hear any dial tone. Normally this is pretty standard, never seen an issue like this before - wondering if it is a hardware issue with the FXS card. Pasted some relevant config below:

I ran into a similar problem where after a period of time the FXS port just locked up even though IOS showed it was fine. What version of IOS are you running?

I se from the show version that the router has been up since

Did you install the card with the router powered up.
Looking at the link
Table 1
POWER OFF ROUTER
If this has not been folowed than the card may be damaged
If you have not already done so I advise you to
"copy run start" and then repower the router at a suitable time
Then try testing the fxs ports again

No I did not. When we originally installed the router, we were unsure if a fax was needed at that time so the card was just left alone. It was determined by the business that they need a fax so I configured the port and....no dice.

I have tried searching for bugs re the FXS ports being stuck in offhook and therefore no dial tone.
Plenty if it were FXO but not relevant to you.
I can only suggest a repower at a suitable time, then retest
Your IOS 15.0.1M6 is still downloadable although M7 is released now too.
You could try an IOS upgrade to M7
If this still fails then a TAC should be raised.

I just tried shutting down the router and moving the FXS card to another slot. Still am not getting any dial tone from any of the FXS ports. It is so weird because during the boot up process I see log messages hat the card was seen and that DSP resources were be allocated accordingly

Based on this thread so far it sounds like you have a DoA card but never knew it. However, there are a couple more things to try first...
Ensure "voice-port 0/1/0" is not shutdown and that there aren't any other non-standard commands on it. In fact please post a copy of the voice-port 0/1/0 config.
So far you've only said you don't hear dial-tone on the port. Have you tried removing the dial-peer voice 10 and applying the directory number 2495 directly to the port like this:
voice-port 0/1/0
connection plar 2495
With or without the dial-peer, what happens if you dial 2495 from another phone? Does it ring?
Turn on 'debug vpm all' and try to take the phone off-hook or dial 2495 from another phone. Attach the results to your next post.

I moved the card in a previous troubleshooting step so the ports in question are now actually 0/3/0 - 0/3/3...

It sure looks like the FXS card is stuck. I tend to agree with Alex's assertion. Upgrade to 15.0(1)M7 and reboot. The only other thing to try ahead of that is doing a shut / no shut on each of the 0/3/X voice-ports and see if that resets them.
If the problem continues, call TAC. You may have a bad card.

*** UPDATE ***
I opened a TAC case and after trying their suggetions (everything I already tried) we ended up change the IOS from 15.0(1)M6 to 12.4(24)T6. This fixed the issue immediately and the FXS card stated acting normal. The TAC engineer did not mention any known bug...........but beware.
