3750-x POE Issue powering Avaya 4620 phone

I have a WS-C3750X-48PF-S switch that will not power up Avaya Model 4620SW IP phones. Following error is displayed:

Hmmmm ... Let's try a TDR cable test ...

1.  Command:  test cable tdr interface Gi1/0/48;

2.  Wait for about 5 to 7 seconds;

3.  Command:  sh cable tdr interface Gi1/0/48;

4.  Please post the complete output.

Pair C controls PoE.  Ideal should be NORMAL.  The patch cord immediately plugged to the 3750X is faulty.  Kindly replace this and let us know if it works.

Pair D controls GigabitEthernet.  OPEN means it's an open circuit or the pin(s) are not touching anything (aka dead air).

No difference with new patch cord. Pairs are open as phone will not power up. Continous retries with the original error listed displayed. Can not find this error listed in any Cisco documentation.

Connect the phone directly to the switch.  IF it still doesn't power up then try the TDR test one more time.

Just for the record:  You replaced the patch cord on the switch-side, correct?

I have been conducting all tests with the phone plugged directly into the switch using different patch cords and testing on 2 different switches (both brand new equipment). Sorry if I did not clarify that earlier.

Pair D as "OPEN"?  Hmmm ...

1.  What IOS are you running?

2.  Can you try a different port?

3.  Is this affecting one phone or all Avaya phones?  Same model?

1) Have tried IOS 12.2(58)SE1 and 15.0(1)SE with the same results.

2) Have tried multiple ports with the same results.

3) Multiple phones of the same model only (4620).

Can't find anything via Avaya support site.

Opened Cisco TAC case and they wanted to RMA the switch without performing any diagnostics. Looks like I am going to have to re-open a case.

Can you do the command "sh post"?

1.  Your IOS is fine.

2.  POST does not show any component failure.

3.  TDR test is consistently showing Pair D (PoE control) as OPEN circuit.  Bizarre.

May I request something?  I just want to determine if your patch cable is faulty or the Avaya phone "responds" differently.

Can you plug patch cord, you are using now with the Avaya phone, to PC or laptop and run the TDR test again?  If the result shows all four pairs as "Normal" then I'm out of ideas.

CDP neighbor is enabled, right?

I actually have the exact same issue as Michael with the combination of these switches and Avaya 4620SW IP phones.  I do have CDP disabled globally since this is part of our security standards.  I haven't ran the TDR test commands yet as you have above but I would like to coordinate this to see if I get the same results.  Does running this TDR test command affect the connected device?

My configuration is:

2 x WS-C3750X-48PF-S each with Redundant 1100W PSU's.  They are configured with Stack-Power in Sharing mode and the switches are Smart Stacked as well?  A "show post" shows that everything has passed as above?  Here is the IOS version I'm running:

Version 12.2(53)SE2

c3750e-universalk9-mz.122-53.SE2/c3750e-universalk9-mz.122-53.SE2.bin

A Cisco consultant had mentioned to me a while back that there was a bug/issue with the PSU's that came with these switches but I have not confirmed that anywhere yet?

I can actually get phones to power up after some time, but it's hit and miss?  I would really like to get this issue solved and I'm willing to help as it seems I'm not the only one now. 

This is the first post I've found directly related to this issue, so at least that's a good sign.
