I have recently come across a problem which I think is hardware related, but I would like some additional input before I get a replacement.
The problem stems from a 4507R running cat4000-i5k91s-mz.122-25.EWA12.bin.
One of the supervisors does not become active and there is no console conversation when booting with this supervisor.

Peer (slot: 1) information is not available because it is in 'DISABLED' state
However, the Trunk link on slot 1 is active and appears to be running ok
I had noticed that the supervisor cpu utilisation light display cyles up to 100% then repeats the process.
We have numerous platforms running this IOS without any problems so I am not convinced that the IOS is causing the problem and coupled with no appearance of any console communication I think this points to a hardware problem.
Any further input would be most useful.

Hi,
Did you manage to progress this? I had the same symptoms on a 4510R but with a single supervisor, meaning we had a full outage for several hours.
I think reseating one of the line cards fixed the issue although a colleague was near the switch when it came back to life. Status light was blank and utilisation light cycling between 1 and 100%, reseted the supervisor several times but didn't fix it, but perhaps the line card did.
Everything on Cisco.com point to hardware issue, possibly redundancy modules and clock module, or maybe just line cards not seated properly.

Hi Paul,
As per the doucmentation and pre-requiste just check the following things :-
1.The same IOS version on both Sup
2.Use the following command by specifying the standby slot number :
3.*hw-module slot* *slot-number *image *file-spec
4.Reload the standby, after reloading it should in hot standby mode.
Try the following things also after performing the above steps:-
hw-module slot 2 image slot0:/rsp-k4pv-mz.120-32.S8.bin
hw-module slot 3 image slot0:/rsp-k4pv-mz.120-32.S8.bin
slave auto-sync config and then (make sure to wr mem first):
hw-module standby-cpu reset

Thanks for your input.
Yes, both supervisors had the same IOS and slot 1 was the ACTIVE supervisor some time ago, at some point it switched over to the standby and made this the Active, as no links went down our monitoring did not report any problems. Currently, I cannot 'see' anything on slot 1 and I am unable to commmunicate with this module. I have already cold booted this module alone and there is no response from the console port or any evidence that the IOS loads.
I can only conclude therefore that this is a hardware/firmware problem.

Hi Paul,
Have you tried taking out the sup and reinserting back in. That some times helps. When you put the Sup back in there, make sure you have console cable plugged in and see if you can watch it as it goes through the boot process.

Thanks for advice.
As this is a 'live' device and with all the processes you have to go through to touch a device, I will try it this weekend.
Howvever, I do not vividly recolect re-seating this particular module, although I'm sure I would have done this.
Having said that, I'll go through both pieces of advice this coming weekend and let you know the outcome.

Hi Andrew,
Apologies for the delay in replying.
We had already re-seatted the line card, tried in another chassis etc etc. Nothing resolved the issue.
It kept dropping into rommon mode and no matter what was done to get the card fully restored it was a waste of time.
The short answer is that we had to replace the hardware module.
