4507R Supervisor Engine IV X4515 not booting

I have a 4507R with a WS-X4515 supervisor V. It was booting into rommon mode, so I attempted to boot to an IOS image from TFTP. The boot process hangs at at the end of the below output and the then the status light blinks orange. I left it sit like this overnight, and it remained the same. Could it be faulty supervisor or am I doing something wrong? Thanks.

I moved the sup to the 2nd sup slot and it is hung at the same place, except this time the status light is RED.

With Sup in Slot1- did you try by setting the config regester to 0x2 or 0x2102? Also try to add this bootflash path..

Ok I moved the sup back to slot 1 and I got the config register set to 0x2102

The  boot=bootflash command didn't work.

rommon 8 >boot=bootflash:cat4000-i5k91s-mz.122-20.EW.bin

Error: 'BOOT' can not be set/unset directly.

You can set it only from IOS.

Type 'clear boot' to clear it

I tried to boot from bootflash and specify the file to boot from.

rommon 9 >boot bootflash:cat4000-i5k91s-mz.122-20.EW.bin

The boot hangs in the same place as before and the status LED is Red.

Oops.. Amber --> red- does not sound good ;-). I would contact TAC (if contract exists) or wait if experts shed some light on this.

Are you sure, you have the correct IOS for this sup?  I don't see cat4000-i5k91s-mz.122-20.EW.bin.  There is only EW4 on CCO see below:

cat4000-i5k91s-mz.122-20.EW4.bin

I am not 100% confident that he image on bootflash, cat4000-i5k91s-mz.122-20.EW.bin is good. However as you see in my first post, I attempted to recover by booting from tftp from a known good image for that 4057R / sup IV and it did the same thing. I just tried it again, with config-register set to 0x2102 and this time the status led is blinking amber.

This sup has been tried in another chassis as well with the same result.

I have opened a TAC case...

I attempte to boot to two other images on slot0 compact flash slot. I loaded them on the flash card and verified the images on another IV sup.

In rommon on the problem sup:

Just for info.. Supervisor IV for 4507R has supported IOS: cat4500-entservicesk9-mz.122-53.SG2.bin. I have one switch in prod with the same Sup & IOS.
