6509 Supervisor 1A continuous failure.

Dear Pros,

In one of our custoner place the cat 6509 switch Standby sup1A continuous failed even after change the hybrid to native.The IOS is C6sup11-jsv-mz.121.26.E3. Please could any one help me to solve the problem. I alreay with cisco TAC.Sample file is attached herewith.
