Catalyst 4510R+E 1000BASE-T Module Stops Accepting New Links

We have a 4510R+E which is configured as follows:

Occasionally, a module in the chassis will stop accepting new links.  That is, a cable plugged into a port on the module, with a working host on the other end, will not result in a link being brought up.  Trying a port on another module works just fine.  Existing connections on the affected module are stable.  If an existing connection is unplugged and plugged back in, the link will not come back up.  Issuing a hw-module module x reset command will reboot the module, and new links will start working again.  We have had three different modules in different slots affected by this problem.

I'm considering a software upgrade, but haven't found reference to any specific bugs that mention this problem.

If anyone has any insight, it would be appreciated.

Could this be a problem of not enough power in the chassis to support the types and number of modules you have?  What does the show power detail command say?

That looks fine.  This is odd since you said that just a specific port won't work while other ports on the same module are working fine.  Moving the cable to a different port and it works fine.  I'd open up a TAC case if no one else here ports anything which might be bug related or hit up the bug search tool and see if you can find anything.

Are there any other error or log messages associated with this behavior?

Other ports on the same module will work fine as long as they are not unplugged (or brought down with a shutdown command, etc.)  The issue specifically seems to be related to the process of bringing the port up.

Unfortunately the log does not show any information related to these events.  A TAC case is definitely my next step, I just wanted to check here first.

According to TAC, we're suffering from this:

CSCts67025

• One or more line cards (WS-X46xx and WS-X47xx series) stop responding to interface changes when near-simultaneous      link events (if timed correctly) occur on the same linecard.  Only line cards in the WS-X46xx series and the WS-X47xx      series are affected. Interfaces that are already linked-up are not affected.

     Workaround: Eliminate link flap. If the problem exists, do one of the following:

– Reload the linecard module with the hw-module module n reset command.

– For dual supervisor engines, perform a switchover. CSCts67025

It looks like I'm also experiencing this bug but I'm running WS-X45-SUP6-E with WS-X4648 and WS-X4548 blades.  I've had the interfaces stop responding to link changes as well as stop providing PoE.  It also caused the CPU to remain at 70% utilization.  I performed the hw-module reset on the problem WS-X45xx blades and that resolved the interface and high CPU issues.  I'm just wondering if anyone else has seen this bug show up on the SUP6-E's and WS-X45xx line cards and what version of IOS 15 is recommended?
