Cataylst 4510 - Avaya Phones rebooting

Anyone using Avaya VoIP phones connecting to WS-X4548-GB-RJ45V blades? Not all phones are doing it, and it happens intermittingly. We have Avaya troubleshooting as well. Any suggestions it will be appreciated! Patch cables are certified. Thanks

It can be a bad blade, have it replaced ASAP.
