cisco 6509 red-switchover issues

I am having issue with a when i tried to my redanduant sup engine switchover manually it goes in to active but then module 1 goes offline and switch goes to rommon mode and I lost connectiviry to switch

someone has any suggestion on this?

Suggestion: Contact Cisco and raise a TAC so you can RMA your card. You may want to attach the TXT file to the case too.

Thanks I have opened up the TAC case with cisco and we have replaced the one of the sup-module
