we haven't rebooted the switch but still logs show it has udergone a coldstart is there any hardware eeror

no error message in bootup logs; first impression is that you got a power cut.

if you have redundant supervisor take also:

where is the rest of the outputs I requested?
I have some ideas of what heppened but I need all the outputs I requested (some of them will not give any info as the sup on slot 5 is not Other - probably in ROMMON - but I could get some valid info from the other outputs I asked you).
You'd better paste all the outputs in a txt file and attach it to this thread.

Slot 5 and 6 are currently occupied by a pair of Sup720. Active card is Slot 6. Maybe Slot 5 Sup720 is running a different IOS? Have you tried to remove and re-insert the card and see what happens?
