Issue with Cisco WS-X6716-10G fiber module

Hi everyone. Hopefully somebody can shed light to this issue we are facing. We are connecting a Dell PowerEdge M620 to a Cisco 6509 core switch via a WS-X6716 10gig fiber module. A few minutes to an hour after connecting it, our entire network goes down. We don't see anything on the logs from the 6509 and we always end up disconnecting it. We have opened a case with cisco TAC already but I would also want to get some opinion from the good people of this forum. TIA.

Correction. We are connecting the core switch to Dell PowerConnect M8024-k Switch, not Dell PowerEdge M620.
