We had following critical error on the Nexus switch on 3rd July which caused full module restart and all fiber links going down. We need to investigate the root cause for this issue and raise support call for the same:
2013 Jul 3 16:24:51 ADDC-Core-Nexus %MODULE-2-MOD_DIAG_FAIL: Module 2 (serial: JAF1432CGNH) reported failure due to Service on linecard had a hap-reset in device 134 (device error 0x282)
Kick start version 4.2.4

Hi,
If your module was reset, you maybe hitting a bug on the image. Open a ticket with TAC and send them the log file.
If upgrading the image does not help, you may have to RMA the module.

It looks like you are hitting bug: CSCtg97646 and not CSCti20899.
This is caused due to a unstable Laser source on one of the ports which is causing a continous interrupts resulting
In a HAP reset. This has been fixed 5.0.3 and later versions.
IF you can isolate and fix the unstable laser source, then it should not
cause this hap-reset to happen.
This is not a hardware problem.
The fix for the issue is available in NX-OS version 4.2(8) and other latest
versions. This defect is triggered by RX LOS on the interfaces causing
excessive interrupts to be raised.
Also, I see that you are running 4.2(4) version. As per the below link, the
recommended version is 4.2(6). So I would suggest to upgrade the box to any
latest code available.
