When should you upgrade rommon? I've never done an upgrade, and I've only upgraded the IOS.

Put simply, when you need to. If you don't need to upgrade the ROMMON to run the software you want to run then leave well alone :-)

The new IOS, 12.4(50)SE series and later, will upgrade the switch's ROMmon to 12.2(44)SE5 (so you don't have a choice whether you want to upgrade the ROMmon or not).
Depending on the routers, I would take advantage of upgrading 1800, 2800, 3800 because there is one major feature I want: USB Boot
Otherwise, I'd follow the old adage: If it ain't broke, doooooooooooooooooooon't fix it.
