This problem can accoured in following situatin:
1.The tftp server is unreachable.
2.Dont run tftp server software.
3.The tftp port in the firewall is closed.
4. The tftp server configuration in transmit only and you must change to transmit and recive.
5.The Windows Firewall in on and tftp port is closed.

Hi,
one thing I want to share in this matter..
The problem is happening during upload only..I am not getting any error when I have taken the IOS or running-config backup.
Answer of your question...
1)TFTP server is reachable from device (pinging from switch)
2)TFTF server is running
3)Windows firewall is closed
4)Please let me know how to check the setting. I am using Cisco TFTP server (Version 1.1) and I didn't get any option to check the same.
5)Windows firewall is closed
