What causes router to boot to this prompt: router(boot)> ?

I was trying to do a password recovery on a 7201 router, IOS=12.2(33)SRC3, and now it boots to a prompt as show in the title. I ran into this a long time ago, but can't remember what the cause/fix was.

config register = 0x2102

If the config register indicates 0x2102, then verify is pointing to the correct location for the IOS.
