WS-X4148-RJ unable to power on at Cat4506 switch

I have a problem with the WS-X4148-RJ. When i insert the module into at Cat4506, this error message pops up

Seeprom Not Programmed

Here is the show module of switch

Chassis Type : WS-C4506

The module is able to work at the other slots...any advice??

This is a hardware failure with the module. I didnt find any bug related to this but I certainly know that this is a hardware failure. Please go ahead and raise a TAC for the replacement of the module. You should be fine with the replacement.

I don't think its a hardware issue for the module. As I have switch the position of the module from slot 3 to slot 6. And the module is working ok. Since you say there is no bugs related to this..i guess the problem lies with the Chasis?

What happens if you move any other module in Slot3? Do you see the same error? If yes, then we can certainly sure that this could be the issue with the chassis, otherwise it will be a module.

Check swapping any other module in slot 3 and update us on this.

I have swap another working WS-X4148-RJ to the slot3. The same error message occurs. This problem started not long ago. Previously it was working fine. Even after taking out the module, the error message is still there when I do a show module. Below shows the message from output interpreter.

ERROR MESSAGE NOTIFICATIONS (if any)

%C4K_CHASSIS-3-LINECARDSEEPROMREADFAILED (x5):Failed to read module [dec]'s

serial eeprom, try reinserting module

Explanation: The switch cannot read the serial EEPROM of the specified module,

and the module will not boot because the EEPROM failed to identify the module

type. The read might fail if the module is not seated correctly in the slot.

Recommended Action: Remove and reinsert the module. If this message reappears,

reboot the switch. If the problem persists, contact your technical support representative.

You will likely have to return the module to Cisco to reprogram or replace the

serial EEPROM.
