WS-X4418-G8 module is faulty on one of WS-4507R-E switches

Hi

we have 2 WS-4507R-E chassis. these modules are installed on each chassis:

module 3:WS-X4516-10GE

module 2: WS-X4418-GB

both chassis has same IOS:cat4500-entservices-mz.122-52.SG.bin

Problem is: on one of the WS-4507R-E chassis when you install WS-X4418-GB after show modull all it says Faulty on status field. But if you remove this module and installed it on other chassis it is ok.

could you tell me what is the problem?

i attached sh version output from both switches and also sh module all output on both switches

Can you try to install the module in the switch it fails in another slot? And
if it still fails capture the output for, "sh log" and "sh diagnostic result module det". We know for sure the module is not faulty, so it may just be a slot issue.

finall i received your requested log. customer said he test module on another slot but problem still consist. you can see on attachment about log reports
